Nurse Care Managers
(Bi-Lingual Skills Preferred)


Boston, New Bedford, Springfield Location 


The Nurse Care Manager will perform a variety of diverse and complex telephonic and face-to-face care management responsibilities in addition to training new staff. Plans, implements and evaluates appropriate care plans and healthcare services in conjunction with the physician treatment plan. You will lead the inter-disciplinary team in the design and implementation of the care plan to enhance the member’s overall health, and to achieve appropriate utilization of services at an appropriate cost. You are responsible to coordinate and/or monitor the medical aspects of care necessary to meet the needs of a specific member population identified as high risk and/or medically complex.




Requirements

Qualified candidates must have a BA in Nursing or Nursing School degree and a current MA license as a Registered Nurse. 3 plus years of related experience in acute care or health insurance with strong healthcare database skills required.



Case Management Certification and experience with Medicaid recipients preferred.
